A-Z Therapy Services is an outpatient facility that provides individualized pediatric services to children, adults, and seniors with a wide range of diagnoses to support performance at home, at school and in the community. At A-Z Therapy
in Rio Rancho, New Mexico we proudly provide occupational therapy services to infants, children, teens, adults young and old, seniors, and the elderly.
We request that you keep all schedule appointments. A 24-hour notice is required for all cancellations or re-schedules. In case of an emergency, please contact us as soon as possible.
Should your missed appointment jeopardize the outcome of your therapy program, you will be discharged and your physician notified. We reserve the right to charge a $40. fee for no-shows, and/or reserves the right to refuse future treatment or services.
The patient is ultimately responsible for the payment for treatment and care, and is required to provide the most current and updated information available regarding insurance.
Patients are responsible for payment of copays, coinsurance, deductibles and all other procedures or treatment not covered by their insurance plan. Payment is due 30 days from receipt of billing. Copays are due at the time of service. Unpaid balances will accrue interest at the rate of 18% APR (1.5% per month). Returned check charge – $30.
